If you are inquiring about a comprehensive evaluation for your child or teen, you are in the right place.

Once you submit this form, our Client Care Coordinator will be in touch to let you know next steps, which may include scheduling, joining our waitlist, or a referral if we're not the best fit.

This HIPAA compliant form takes about 5 minutes and helps us understand your needs so we can get the process started on our end.

Before you start, please keep in mind:

• Insurance: We are out-of-network with insurance, but your plan may reimburse part of the cost. The total cost for a comprehensive evaluation typically starts at about $3500, with most evaluations landing in the $4000 to $5000 range. • Mandated reporting: We're required by law to report suspected abuse or neglect of a minor or vulnerable adult, even when communicating with non-clients.
