If you are inquiring about a comprehensive evaluation for your child or teen, you are in the right place.

Once you submit this form, our Client Care Coordinator will be in touch to let you know next steps, which may include scheduling, joining our waitlist, or a referral if we're not the best fit.

This HIPAA compliant form takes about 5 minutes and helps us understand your needs so we can get the process started on our end.

Before you start, please keep in mind:

• Insurance: We are out-of-network with insurance, but your plan may reimburse part of the cost. The total cost typically starts at about $3500, with most evaluations landing in the $4500 to $6000 range. Costs can exceed this range, depending on the provider and time involvement. Please review our FAQ and Billing FAQ for additional details about fees and insurance related questions.

• Mandated reporting: We're required by law to report suspected abuse or neglect of a minor or vulnerable adult, even when communicating with non-clients.
