Once you submit this form, our Client Care Coordinator will be in touch to let you know next steps, which may include scheduling, joining our waitlist, or a referral if we're not the best fit.

This HIPAA compliant form takes about 5 minutes and helps us understand your needs so we can get the process started on our end.

Before you start, please keep in mind:

• Insurance: We are out-of-network with insurance, but your plan may reimburse part of the cost. Session fees are $215 to $240, depending on the provider. Please review our FAQ and Billing FAQ for additional details about fees and insurance related questions.

• Mandated reporting: We're required by law to report suspected abuse or neglect of a minor or vulnerable adult, even when communicating with non-clients.
